Research and Training Data Consent

IDD Mentorship, LLC

IDD Mentorship is committed to improving the care we provide and to contributing to the broader understanding of how integrated mentorship serves the human person. To do this, we collect and study data from our mentorship work. Your participation in this research is entirely voluntary. All information we use is separated from anything that gives any specific data about who you are.

What we are asking permission to use:

Information you provide during intake and throughout your mentorship
Content from your Voxer messages with your mentor
Your mentor's notes and reflections about your work together
Any follow-up questionnaires you choose to complete

How your data is used and protected:

Before any of your data is used for research or training, all identifying information is removed. This includes your name, contact information, specific names of people in your life, identifiable locations, and any other details that could connect the material back to you.

De-identified data may be used for:

Research on the effectiveness of integrated mentorship
Publications, presentations, and academic writing
Training and supervision of current and future mentors
Educational materials including case studies, podcasts, articles, and course content
Program improvement and quality assurance

Identifiable data, before de-identification, is stored securely and accessed only by authorized IDDM staff for the purpose of administering this research.

Your choice does not affect your mentorship. Whether you consent or decline, your mentorship experience, fees, and your mentor's care for you will not change. Your mentor will not know your decision unless you choose to discuss it.

You may withdraw at any time. You may withdraw your consent by emailing [email protected]. Withdrawal stops any future use of your data. Data that has already been de-identified and incorporated into research or training materials cannot be retrieved, but no new data will be collected or used.
